Unique fields

Some field types only admit a single instance per form. Here is a list and the reason:

Name
Email address
As they are special fields that identify the user identity, one instance should be enough, but you can collect extra names and email addresses, simply using additional text fields.

Attachments
There is no need to have multiple buttons to obtain multiple upload. Just do a multiple selection, or press the upload button multiple times.
Have you ever seen several attach buttons in a row on Microsoft Outlook?

Captcha
One captcha is annoying, multiple captcha would be insane.

User Information
Newsletter Integration
Those are special hidden fields whose content is automatically filled out by Fox contact. Multiple instances would always hold the same value.
