I had to create a second name field called "surname" since many users were not filling the contact form with their complete name and this was causing confusion in our records. The problem is, now when I get an email notification it only includes the users name, and not his surname and this is also causing some confusion.

Could you add the option to have 2 fields that are included into the email subject? This would also be useful for users who wish to have the users company name included into the email subject or other sorts of quick info.
